A 24-year-old G2A1 at 34 weeks of gestation was admitted with complaints nausea, vomiting, pedal edema and high blood pressure recordings. She developed imminent symptoms after admission for which she received prophylactic magnesium sulphate therapy a provisional diagnosis severe preeclampsia made. With worsening hematological, liver renal parameters as did not fulfill the Swansea’s criteria acute fatty pregnancy (AFLP), partial hemolysis elevated enzymes low platelet count (HELLP) suspected. delivered by cesarean section. Infectious disease work up sent in view rising counts positive leptospirosis. There also history walking rice fields bare foot rat infestations supporting diagnosis. Both mother baby were discharged healthy condition. The becomes challenging it mimics induced hypertension, pregnancy, HELLP, obstetric cholestasis viral hepatitis.
